· Must be a team player and assist PCA, LVN and secretary with incoming phone calls, paging personnel, taking vitals signs when necessary.

- The Medical Intensive Care Unit on 9F, 10C, 10F and 11F is a 32-bed intensive care unit with the primary focus of caring for patients from the pulmonary, pulmonary transplant, medicine, gastrointestinal and toxicology services.
- The Cardiac Intensive Care Unit on 3F is a 10-bed intensive care unit serving critically ill coronary patients. The population consists of young adults through geriatric patients.
- The Cardiothoracic Intensive Care Unit is a 20-bed intensive care unit that is one the largest Cardiothoracic ICUs in the city. Patient population includes all open-heart surgery patients, including heart and lung transplant patients, high-risk thoracic surgery and vascular patients.
- The Surgical Intensive Care Unit, including 2D, is a 14-bed surgical critical care unit specializing in the care of a wide variety of critically ill surgical patients. The surgical services utilizing this unit include vascular surgery, which is the primary service, thoracic, trauma, neurosurgical, renal, and general surgery.
- The Surgical Trauma Intensive Care Unit on 6F and 6G is a 22-bed surgical trauma unit caring for all trauma patients and general surgical patients.
- The Neurological Trauma Intensive Care Unit on 4G is a 10-bed surgical critical care unit serving the adult and geriatric trauma patient populations. The primary focus is the care of the critically injured trauma patient with head trauma but also includes those patients sustaining chest and abdomen trauma, spinal cord injury and orthopedic trauma.
- The Neurological Intensive Care Unit on 4F and 5F is a 20-bed intensive care unit with the primary focus of caring for neurovascular patients including subarachnoid hemorrhage, intracerebral bleeds and stroke.
- The Transplant Intensive Care Unit on 5 South, 5 East and 5 West is a 28-bed ICU that specializes in abnormalities of the liver including Liver Transplantation (pre- and post-operative care). Other transplants cared for in this unit are Small Bowels, Kidneys, and Kidney/Pancreas.

· Demonstrates the knowledge and skills necessary to provide care that is relevant to patients under their care and applies principles of growth and development over the life span
· Possesses the ability to assess and interpret patient data needed to identify each patient's requirements relative to his or her age specific needs

Description:
The per diem care coordinator will assist with care coordination for lung transplant recipients as outlined:
1. Conducts inpatient care coordination for all lung transplant patients, including participation in inpatient rounds, documentation in inpatient medical record, daily care facilitation and communications, utilization review and management, organizational efficiency and discharge planning.
2. Acts as liaison between members of inpatient care team, including physicians, trainees, physician assistants, etc, and outpatient care coordination team, including outpatient nurse coordinators.
3. Facilitates access to and efficiency of outpatient care by direct patient management in clinic setting in conjunction with attending physicians, documentation in outpatient medical record, and integration of inpatient and outpatient services.
4. Participates in monitoring patient progress after discharge to the outpatient coordinators by facilitating transfer of communication with patient/family, other team members, providers and agencies to the outpatient coordinators.
5. Coordinates seamless transition of care between in and out-patient care by working closely with out-patient lung transplant coordinators.
6. Works with the financial coordinator, social work and other means of assistance as needed.
7. Communicates patient information to the care team via appropriate documentation.

Staff Nurse opening on our Blood and Bone Marrow Transplant Unit. 60 hours every two weeks, Rotating Shifts, 7am-7pm,every third weekend. 3 years pediatric oncology experience preferred.
This is a 10-bed Blood & Bone Marrow Transplant (BBMT) unit. To provide comprehensive care for patients undergoing Blood & Bone Marrow Transplantation, the unit was designed with all private patient rooms. There are also two outpatient/procedure rooms on the unit where BBMT outpatients are evaluated and treated. Additionally the nurses on this unit provide care for inpatients requiring services from the Hematology/Oncology subspecialty. Admitting diagnoses include Leukemia, brain tumors, neuroblastomas, solid organ malignancies, sickle cell disorders and other types of genetic blood disorders. In addition, adult stem cell and bone marrow donors will be cared for on this unit.
The level of patient care provided in this area is complex due to patient diagnosis, the intensity of therapy, the criticality of patient condition, and the extensive patient/family issues that necessitate nursing intervention. We employ the primary nurse model for care delivery. The primary nurse is responsible for coordinating patient care as part of an interdisciplinary team. Nursing competencies required in this area are care of the pre, ante and post bone marrow transplant patient including the administration of chemotherapy, as well as in the care of a patient receiving chemotherapy. Competencies are accomplished and maintained through educational programs provided by Nursing Education
